[FAB-1524] Reinitialize chains on orderer restart

This changeset utilizes the last configuration embedded in the block
metadata to properly initialize any existing chains.

It also fixes the template based configuration transaction creation in
the utilties to omit the ChainCreators configuration item which
inappropriately flags a chain as an ordering system chain.
